You are being asked to be in a quality improvement (QI) project. This sheet will tell you more about this project, and the information being requested from you. You can also find contact information if you have any questions or concerns about this QI project.
Why is this QI project being done?
QI projects can help ensure programs, like the heart transplant support group, are performing well. The purpose of this QI project is to identify relative strengths and areas of improvement for the heart transplant support group to engage in a continuous cycle of improvement.
Your participation will help us ensure that the program is serving the heart transplant community successfully, by identifying what works and what needs improvement.
What happens to me if I agree to participate in the QI project?
If you decide to take part, the general procedures include completing a brief survey at the end of each support group session. This survey will ask you questions about whether you’d participate in the group again, what you liked most about the group, whether you learned new skills, whether the skills were explained well, and if you had any questions, concerns, or feedback. We expect that your taking part in this QI project survey will take approximately two minutes to complete.
There is no private or identifiable private information being collected in this QI project.
Your participation is voluntary. You may decide not to participate, or you may choose to only respond to some (not all) survey questions.
Your alternative is to not participate in this QI project. Your decision will not result in any penalty or loss of benefits to which you are otherwise entitled. This includes your ability to participate in the heart transplant support group.
Could completing this QI project survey hurt me?
There are no known risks associated with being in this QI project.
Will completing this QI project survey benefit me?
Your feedback will allow us to make changes that could improve the support group to better fit your needs, and this will allow the support group to provide more tailored services. Additionally, others in the support group may benefit from the information obtained from this research.
What happens to the information collected from this QI project?
Information collected from this QI project will be used to inform improvements to the support group. We may also publish or otherwise disseminate (or share) the results of this QI project to the community.
